We have a robust inpatient consultation service, including the cognitive behavioral service focused on addressing delirium and dementia in older adults admitted to the hospital under medicine and surgical services. 

Our geriatric medicine consultation service provides comprehensive geriatric assessments for patients admitted to trauma and other surgical services. Our goal in these cases is to address geriatric syndromes with a focus on the 4 Ms of Age-Friendly Care:

• Mind / mentation (delirium, dementia)
• Mobility (falls)
• Medications (polypharmacy)
• What matters most (what our older adult patients prioritize in their lives)   

Additionally, our team helps co-manage older adults who are admitted under surgical services, including hip fracture patients and those undergoing elective procedures. The consult service also provides medical consultations for patients who are currently admitted to the acute care inpatient rehabilitation center.
