Psychogeriatric Assessment and Treatment in City Housing (PATCH)
The PATCH Program is a nurse-based service that provides mobile treatment services to elderly people with serious and persistent mental illness who live in city housing for seniors. Services include in-home psychiatric assessment and treatment, improvement and coordination of community services and education for caregivers about special needs of the elderly. The program has been operating since 1986 in conjunction with the Housing Authority of Baltimore City.
