Proxy Access to a Child's MyChart Information (age 0 to 12)

Depending on the child's age, parents and guardians have different levels of access to their child's MyChart information:
Full Access: 0 to 12 Years Old
Parent (Proxy) Access to a Teen Patient’s MyChart Information (age 13 to 17)
Parents (proxies) and teen patients have different levels of access to teen health information in MyChart accounts. This is because of regulations protecting a teen patient’s ability to seek confidential health care. Johns Hopkins Medicine has been working to improve what can be seen in proxy accounts so that parents of teen patients have access to the information they need to navigate our health care system alongside their teen child.
Access for parents and guardians of teen patients includes the ability to:
- Schedule and view most appointments.
- View most notes and other information about visits.
- View most vaccination records.
- Send secure messages to clinicians.
- Pay bills (guarantors only).
The following abilities are coming soon.
- View most lab results.
- View and request refills for most medications.

Adult Patient Authorizing Access to Their Own MyChart Account (age 18-plus)
New to MyChart? If you are requesting proxy access another person's MyChart account, you will need to create your own MyChart account at Johns Hopkins. It is easy to create a Johns Hopkins patient record for yourself. Sign up online.

Adults (age 18 and older) can authorize another person to access their MyChart information to help with their health care needs. Proxy access can be stopped at any time by the proxy or the patient.
Levels of Adult Proxy Access:
- Full Access: Access to all MyChart features.
- Scheduling Only: The ability to view and schedule appointments only.
- Incapacitated Adult: For adults who are not able to make their own medical decisions, proxy access can be provided to the legal representative.

Incapacitated Child Proxy Access
For a child who is not able to make his or her own medical decisions, full access can be provided to the child's legal representative. This is called incapacitated child proxy access. Download and print the Incapacitated Child form. This form must be submitted at a Johns Hopkins office along with legal documentation and verification from two medical physicians.
Incapacitated Adult Proxy Access
- Download and print the Incapacitated Adult Proxy Access form.
- Complete and sign the Incapacitated Adult Proxy Access form and submit it to a Johns Hopkins staff member along with documentation supporting your legal representative status (court order or other legal document).
- Once the legal documentation has been reviewed and approved, Johns Hopkins staff can complete the proxy set-up process and create your MyChart proxy account.
- Johns Hopkins can provide you with the access code necessary to set-up your MyChart account.
