We want every moment to count during your clinic appointment with us. Please be sure to bring (or fax in advance) any updated labs or scans you may have completed that are relevant to your appointment. Please also make sure to bring an updated list of your medications, insurance card and referral (if needed). 

If you are unsure as to whether we have received your records in advance by fax, please call 410-550-2565 or send us a MyChart message for confirmation. It is always a good idea to bring what records you have, as well.

Please plan on arriving 30 minutes early to your appointment to allow for parking, registration, insurance verification, medication intake and vital sign measurement. 

Please also take the time to print out and complete a new or return patient questionnaire that we can review with you in clinic (PDF downloads below).
