The Johns Hopkins Comprehensive Transplant Team

The transplant process is complex and involves a multidisciplinary team along every step.

The Johns Hopkins Comprehensive Transplant Center (CTC) is composed of surgical faculty, medical faculty and staff dedicated to the transplant patient population from the point of referral for transplant through survivorship. The CTC has team members dedicated to our solid organ transplant groups and our adult- and pediatric-specific programs.

Our kidney and liver transplant teams include advanced practice providers, nurse coordinators and patient services coordinators dedicated to the pre-transplant patients, post-transplant patients and live donors. Our team also includes an experienced and specialized group of social workers, pharmacists and dietitians. These teams work with our faculty to manage the medical, psychosocial and other needs of our patients throughout the transplant continuum and help ensure a seamless transition throughout the various phases of transplant care.

The abdominal transplant team also includes highly experienced inpatient advanced practice providers who manage our inpatient population. These patients are cohorted on the Comprehensive Transplant Unit (CTU), which is a 26-bed unit that serves pre- and post-liver and kidney transplant patients and live donors. Our advance practice providers coordinate a multidisciplinary approach to patient care through collaboration with social work, pharmacy, nutrition, hepatology, nephrology and surgery present during daily morning rounds. The inpatient advance practice providers provide coverage seven days a week. In addition, there are surgical residents who rotate on the transplant service and assist with the perioperative transplant care.

The CTC also has four on-call nurse coordinators who are committed to assisting with the coordination of organ offers, organ acceptance and the administrative aspects of surgical planning.
