To schedule an appointment or refer a patient to our center, please call 410-955-5810. Please prepare or have your physician put together the following information:

• Medical history
• Diagnosis
• Any tests or procedures that have been completed related to their diagnosis.

When you call, we'll let you know how to send these documents to our clinic coordinator. After we examine your records to determine which physician is most suitable to treat your condition, we'll contact you to schedule an appointment and send you a new patient package. We ask that you complete the new patient questionnaire and bring it to your scheduled visit.

Before Your First Appointment

If this is your first visit, please bring:

1. Relevant physician records
2. Lab reports
3. X-ray films
4. CT or MRI films (If available)
5. Electromyography (EMG) (if available)
6. Completed new patient questionnaire

If you have been referred by a physician, please bring the complete name, address, and phone number of your referring physician.

We ask that you arrive 15 minutes prior to your scheduled appointment time to complete any necessary registration information.

If you must cancel this appointment, please call us at 410-955-5810 at least 48 hours in advance. We will happy to schedule your appointment at another convenient time.
