Patient Registration
The registration process helps us update your current information and ensure your address, birth date, insurance and other information are correct so your medical record and bills are handled smoothly and accurately. Please be sure to provide the complete insurance information for all of your plans during registration. You may also need to pay deductibles or co-payments at registration.
Register Ahead of Time
Johns Hopkins hospitals allow patients to register before the admission date to save time at admission. Once you are scheduled for an outpatient procedure or an inpatient admission, a representative from your hospital’s Admissions Department will contact you to verify your information.
Please be prepared to provide:
- Your full name (exactly as it appears on your driver's license and/or government-issued ID)
- Address
- Telephone numbers
- Primary and secondary contact information
- Complete insurance information from all of your plans
- Name and telephone number of your primary care physician
Many procedures require insurance preauthorization, which needs to be completed at least two days before your scheduled procedure. If you are scheduled for surgery, you will receive an additional call from a nurse to discuss your procedure before your appointment.

Fax: (202) 243-2246If you are unable to register in advance, allow extra time to complete the registration process when you arrive at the hospital. On the day of your admission, please register and/or check in at Direct Admissions between 7:30 a.m. and 4 p.m.
Maternity Preregistration
You can save time when you are in labor by preregistering before your expected due date for delivery. You can pick up a preregistration form from your physician’s office, at a hospital prenatal tour or at Admissions in the Sibley Memorial Hospital main lobby.

Most insurance plans require pre-authorization for your admission for the delivery of your baby. Check with your employer’s benefits office or insurance company customer service representative to make sure you have met their notification requirements.
If you are unable to complete the pre-registration forms in advance, you and your support person will need to allow extra time to complete the registration process when you arrive at the hospital. Please bring your insurance cards and photo ID for us to copy.

Please note that patients must register at the Main Registration in addition to completing this form. Pre-registration is not available for the emergency department or the behavioral health unit.
You may also pre-register by phone by calling the pre-registration department at 301-896-2222. Please let us know if you are deaf and require a sign language interpreter so we can arrange services for you.

OB patients are required to check-in with the Admissions Department in the hospital lobby. OB patients may pre-register with their doctor or midwife's office in advance of their due date.
Medical-surgical patients are required to check-in at the Berman Pavilion, located to the right of the hospital main entrance.If your doctor has ordered pre-surgical testing, contact our Outpatient Testing Center at 410-884-4884 at least three days prior to surgery to schedule tests.
When registering, please discuss any special needs with an Admitting counselor. Special accommodations are available for patients with disabilities. For patients who are hearing impaired, in-room TDD equipment and closed-captioned televisions are available.
