Preparing For Your Appointment

We encourage you to write down the questions you want to ask our doctors. Consider bringing a family member or friend to your appointment. This companion can help you travel to the hospital, listen and ask questions during the consultation, and offer support in making decisions about brain tumor care.

Please bring to your appointment:

  • The appointment information and materials sent in the orientation packet
  • Health insurance information including insurance card(s), special forms, referrals or important phone numbers
  • Any additional medical records not already sent in advance of the visit
  • A list of your physicians, with their names, addresses and telephone numbers
  • A list of any medications you are currently taking along with the prescribed dosages
  • A pen and notebook and/or folder for keeping a list of questions to ask the doctor, taking notes and storing new paperwork or educational materials from the visit

Patients from out-of-town should also bring contact information for where they are staying in Baltimore.

For Neurosurgery Consultations

When you arrive, please provide your new patient questionnaire if one was sent to you in advance. If you did not receive one in the mail, you can download the questionnaire here. If you do not bring the completed questionnaire to your appointment, you will be asked to complete it upon arrival. Please also bring all recent MRI scans relevant to the brain tumor (please bring the actual images rather than CDs if possible).

For Oncology Consultations at the Kimmel Cancer Center

Please review information from Kimmel Cancer Center Patient and Family Services on preparing for your appointment.
