Appointments
When you first come to the office you will be asked to fill out an information packet. A copy will be made of your insurance card and ID. As a courtesy, we will file insurance forms for you. Payment for services is expected at the time of service.
Patients are responsible for arranging pre-certification/authorization for the office visit. If diagnostic studies (X-rays, etc.) have been done prior to your visit, the actual films or disc are needed to complete the evaluation. Copies of pertinent records should be brought as well.
Please keep appointments. If you are unable to come to a scheduled appointment please notify the office at least 24 hours in advance. Keeping appointments is critical in providing good care for your child as well as our other patients. Write down questions you have about your child’s condition or care before coming to your appointment.
Your pediatrician coordinates your child's care. For general health concerns, please contact your pediatrician. Your pediatrician is in the best position to decide if specialty consultation is necessary.
It is best to call before coming to the Emergency Room, unless you feel it is an immediate, life-threatening condition.
