Adult Targeted Case Management

The Adult Targeted Case Management Program provides coordination of mental health services for individuals in the East Baltimore community who meet criteria defined by the Maryland Department of Health and Mental Hygiene.

Case managers help participants who have a chronic and persistent mental illness access needed community services while encouraging them to live as independently as possible. The program provides home visits, entitlement procurement, assistance with housing, service coordination, help with medication, wellness education, and linkage with needed resources. Services are currently available for Baltimore City residents and eastern Baltimore County residents. All participants must be authorized for service by Maryland's Behavioral Health Administrative Services Organization.
