Appointments & Referrals
The Johns Hopkins Melanoma/Skin Cancer Program is known for its roster of skin cancer experts representing a wide variety of specialties. Appointments for patients already diagnosed with skin cancer can be made through a single-access point to facilitate both physician and self-referrals.
What if I have a diagnosed melanoma or non-melanoma skin cancer?
Call 410-616-7660 to make an appointment with an expert in our Melanoma/Skin Cancer Program. A downloadable Appointment Request Form is available and can be faxed to 410-616-7663.
Information that will be needed to make an appointment includes:
- Patient’s contact information, brief medical history and insurance information
- Pathology reports
- MRI, CT, PET scan and lab results (if these tests have been done)
- Surgical operative notes, if any
- Referral from primary care physician (PCP) if your insurance requires it
We also need to have our pathologists review your pathology slides, to confirm your diagnosis prior to the appointment.
We understand that this process can be overwhelming. Our scheduling team will help you obtain all of the above and will provide clear, detailed instructions to assist you in making the arrangements.
