At the Johns Hopkins Healthful Eating, Activity & Weight Program, our goal is to be a resource that provides a patient-centered experience for the delivery of multidisciplinary, evidence-based clinical services to prevent or manage chronic diseases through lifestyle changes and weight loss for your patients. We are committed to providing services supported by robust scientific evidence.
We believe in a collaborative patient management approach with referring physicians. Our physicians do not assume the primary care of your patients.
Referrals
Referral to the Johns Hopkins Healthful Eating, Activity &Weight Program may be appropriate for patients with the following conditions or needs:
- Overweight/Obesity
- Prediabetes
- Hypertension
- NAFLD
- Women’s Health Conditions including PCOS, Menopausal Weight Gain, Infertility, Lipedema, and Breast Cancer Survivors
- Perioperative Care for Bariatric Procedures

How to Refer
For providers within Johns Hopkins
Place a referral in Epic using the “Referral to Internal Medicine” order, then select the button for the Healthful Eating, Activity & Weight Program and the indication for referral. Central scheduling will reach out to your patient to schedule an initial appointment. If you have any questions about the internal referral process, please call 410-616-7420.
For providers outside Johns Hopkins
Please fill out the Physician Referral Form and fax to the Johns Hopkins Healthful Eating, Activity & Weight Program at 443-367-3215. If the patient’s insurance company also requires a specific referral form, please fax that as well. We will contact the patient to schedule an appointment.
